The following forms are needed to start school.

- Copy of Birth Certificate
- Child Medical Statement
- Immunization Record
- ATC Emergency Medical/Transportation Authorization
- Child Medical/Physical Care Plan (if requiring medication)
- Kindergarten Assessment Form (Kindergarteners coming from a different school)
